What you’ll do
- Receive and return patient calls, perform protocol-based triage where appropriate, and route phone notes to providers.
- Create provider-directed calls and template letters about lab results.
- Prepare records for upcoming visits by confirming that relevant results and care-transition information are available.
- Contact patients, consultants, imaging centers, and hospitals to obtain missing documentation.
- Call patients who may miss appointments and support rescheduling and referral needs.
- Begin forms for provider review, including disability, home-health, equipment, supply, and disease-status requests.
- Help care teams complete population-health reports and identify patients who may need social-service or community referrals.

- Ability to work a three-day, 12-hour Central Time schedule from 7:00 a.m. to 7:00 p.m., Monday-Wednesday or Wednesday-Friday.
- Ability to travel quarterly to Houston, Texas.
